Support at Home provides tailored care packages based on what you need to live comfortably and independently in your own home:
What Care and Support Can You Receive?
Your package is built around your individual routine and preferences. Depending on your needs, services can include:
End-of-Life Care (Palliative Support)
For older adults diagnosed with a terminal illness (three months or less to live) who wish to stay surrounded by the comfort of home, the End-of-Life
Pathway
offers extra support. Working alongside specialised palliative care teams, this pathway provides gentle, dignified care for daily living,
personal needs, and clinical health during life’s final stages.

This program is designed to help older people purchase equipment or small home modifications needed to live safely and comfortably at
home.
What does this mean for you?
If you need extra support to move around safely or make your home more accessible, dedicated funding is available alongside your care package:
You can talk to us about what would make your home safer, things like rails, ramps, or assistive equipment to help with daily home
activities.
If needed, we can arrange for an Occupational Therapist (OT) to visit your home, assess your needs and recommend the right supports.
Your Care Manager will guide you through this process and can apply for this extra funding on your behalf whenever the need arises.

What does the Consumer Advisory Board do?
It brings consumer experiences directly to our Governing Body and management team to ensure our care remains safe, inclusive, and tailored
to our community. The Consumer Advisory board contributes to;
1. Advising on organisational strategy, governance, and service improvements.
2. Reviewing client tools, resources, and risk management plans.
3. Ensuring our care reflects the diverse needs and voices of the people we support.
We are always looking for diverse client perspectives to help guide our journey.

We love bringing our community together at Holistic Horizons, and one of the ways we do this is by hosting regular community barbecues. We welcome everyone – staff, participants, community members, local businesses, family, and friends – to join us for a fun and relaxed afternoon.
Our community barbecues are a great opportunity to catch up with friends, meet new people, and enjoy some delicious food. We provide a
sausage sizzle and drinks, and we cater to all dietary requirements, with vegan, vegetarian, and gluten-free options available.
At the core of our methodology lies inclusivity and collaboration, centered around compassion and self-empowerment. Our aim is to assist
people with disabilities to live their best life possible, and we take pride in the positive outcomes we’ve achieved with our clients and
their families. At the heart of everything we do is a deep sense of caring and understanding, which underpins our commitment to excellence.
We host a Quarterly Network Breakfast and it has been a great success, with attendees from across Moreton Bay Region and North
Brisbane. Our events feature guest speakers who lead informative discussions on topics relevant to Support Coordinators, Plan Managers,
and Disability Services. Join us to expand your knowledge, connect with industry professionals, and engage in meaningful conversations.
